About the Role
Provide compassionate home health care that empowers patients to live safely and comfortably at home. Coordinate care, support families, and mentor nursing staff.
Responsibilities
- Deliver and coordinate patient care in alignment with physician orders
- Supervise and provide ongoing support to LPNs and caregivers
- Develop individualized care plans with input from patients, families, and providers
- Educate and support families in understanding diagnoses and treatments
- Ensure accurate, timely documentation and communication
- Participate in on-call rotation to provide patient and staff support
Requirements
- Graduate from accredited school of professional nursing
- Current RN License in NYS and registration
- Valid driver's license
- Commitment to compassionate, patient-centered care

Salary/Compensations
- $81,120-$87,360
