About the Role

FOX Rehabilitation is seeking passionate, licensed Physical Therapists in Rye, NY, to provide exceptional one-on-one care to older adults in their homes or senior living communities. This role focuses on improving patient mobility, independence, and quality of life through the Geriatric House Calls™ model, offering autonomy, flexibility, and meaningful patient outcomes.

Responsibilities

• Provide one-on-one physical therapy to older adults in private residences or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions supporting patient wellness, longevity, mobility, and functional independence.
• Document effectively within FOX’s EMR system.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
• Provide telehealth / virtual care when appropriate.

Requirements

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.
• Ability to meet physical demands of in-home and community-based care, including lifting, patient transfers, standing/walking for extended periods, climbing stairs, and performing therapeutic activities.
• Ability to drive to patient homes and senior living communities.
• Ability to wear PPE when required.

Salary/Compensations

• Full-Time: $75,000 - $115,000 annually
• Part-Time and FOX Flex (PRN) will be pro-rated.
