About the Role
The Community Hospice has a wonderful opportunity for a Per Diem LPN to work as a member of our Hospice Home Care Team caring for patients and families in both the homecare and nursing home settings. Primary region to cover is Saratoga and Washington Counties. Needs would be on the WEEKDAYS, no weekends.
Responsibilities
- Coordinate closely with our RN Supervisor and RNs to provide nursing and personal care services as part of a comprehensive plan of care to provide added support to patients and families
- Training provided in end of life care.
Requirements
- Valid NYS LPN License
- Minimum 1 year LPN experience required
- Prior hospice/ home care experience preferred, but not required
- Strong computer skills
- Organization and communication skills and ability to work as part of a team
