About the Role

A key component of this job is to support and promote residents' independence by establishing care goals and teaching residents to understand their condition, medications, and self-care skills.

Responsibilities

  • Performs nursing assessment of residents upon program entry and annually thereafter, identifying risk factors and providing care for presenting medical and/or psychiatric issues.
  • Assesses the health needs of all recipients and makes appropriate medical and mental health referrals.
  • Assists with medical and/or psychiatric emergencies immediately upon notification and follows up to ensure recovery.
  • Participates in the development of residents' Individual Support Plans (ISPs).
  • Maintains accurate, detailed reports and records symptoms or changes in residents' conditions.
  • Documents encounters with residents on a progress note and files notes in a timely manner.
  • Interprets and evaluates diagnostic tests to identify and assess residents' condition and shares information with the team.
  • Consults and coordinates with residents' Psychiatrist/behavioral health and healthcare providers to assess, plan, implement, or evaluate residents' care plans.
  • Monitors all aspects of residents' care, including diet and physical activity.
  • Assists clients with pre-packing medications and education around side effects as needed.
  • Facilitates groups for residents on topics such as health education, medication management, symptoms of mental health, and disease prevention.
  • Assists residents and service staff to develop health improvement programs to promote good health practices.
  • Maintains nursing supplies inventory by checking stock, anticipating needed supplies, and placing/expediting orders.
  • Some evening and weekend work may be required.
  • Performs other duties as required or assigned by supervisor.

Requirements

  • Must have strong written and verbal communication skills.
  • Must have knowledge of medical and psychological terminology.
  • Must provide good customer service, both in person and by telephone.
  • Must possess good organizational skills.
  • Must have a high attention to detail.
  • Must have the ability to handle multiple tasks.
  • Must be able to prioritize work assignments.
  • Must maintain confidentiality.
  • Must have at least 2 years of experience working with substance abuse, mental health, and homeless populations.

Skills

  • Bi-lingual Spanish a plus
