About the Role

An Intermediate Care Facility (ICF) is a community-based home providing clinical and supportive care to individuals with intellectual and developmental disabilities (ID/DD) who typically require long-term care and have complex medical or behavioral needs. Clinical supports are provided on-site by an interdisciplinary team and the facility is regulated by the NYS Department of Health.

Responsibilities

  • Assist residents with personal hygiene needs including washing, toileting, feeding, and showering to maintain hygiene and enhance independence.
  • Transport residents to medical appointments, if applicable.
  • Perform light housekeeping tasks to ensure facilities are clean.
  • Implement resident-specific programs by following goal book outlines to ensure independence and life goals.
  • Assist residents in adhering to prescribed diets by ensuring correct portions are dispensed.
  • Administer medication and medical procedures if qualified (AMAP certified) by following prescriptions and doctors' orders.
  • Maintain resident safety through ongoing supervision and utilization of safety techniques and adaptive equipment.
  • Provide ongoing supervision by being in close physical proximity to residents.
  • Implement recreational activities such as arts and crafts, playing tapes, setting out board games, and playing basketball.
  • Serve as a member of the inter-disciplinary team by attending and participating in meetings to discuss resident progress and goal achievements.
  • Purchase clothing, grooming, and other personal items to meet residents' daily living needs and requests.
  • Train new employees by covering the training manual and demonstrating direct care skills, if applicable.
  • Perform additional duties as requested by supervisor.

Requirements

  • High School Diploma or Equivalent preferred.
  • A valid NYS driver's license is required and must be maintained throughout employment.
  • Must meet agency insurance requirements.
  • Six (6) months of experience working with the DDI population preferred.
  • Ability to understand written and oral directives/information.
  • Physical aspects may include lifting and transferring of an adult/child participant.
