About the Role

Supervises and works directly with direct support professionals in the delivery of care to residents, focusing on goal implementation and monitoring. Provides feedback to staff to ensure quality resident care and assists with maintaining residents' personal needs and safety.

Responsibilities

• Supervise direct support professionals in implementing person-centered program plans through observation and review.
• Assist with training and orienting new direct support staff to residents and responsibilities.
• Ensure all staff are trained on program plans and required annual training, including incident reporting, abuse prevention, SCIP, and CPR.
• Conduct daily checks and monitoring of direct support professional data, including goals, behavior plans, and mealtime data.
• Assist with completing and ensuring accuracy of daily assignment sheets.
• Assist with ensuring all community outings and medical appointments are fulfilled daily.
• Provide direct support to individuals by assisting with daily living skills and ensuring personal needs are met.
• Ensure the safety of individuals by following safety precautionary measures according to person-centered plans.
• Provide in-service training to new employees on work rules and procedures.
• Attend semiannual and annual meetings for individuals to evaluate program effectiveness and discuss potential changes.
• Ensure all agency policies and procedures are implemented and enforced.
• Perform additional related duties as requested by supervisor.

• Ensure work-related safety procedures are in place and adhered to by staff.
• Immediately report unsafe acts or potentially hazardous work environments to a supervisor.

Requirements

• High school diploma plus six months of related experience with the I/DD population or a college degree in a health service-related field.
• Must submit and maintain a valid and qualified NYS driver's license.
• Physical aspects may include lifting and transferring of an adult/child participant.
