About the Role
Join the exceptional care team at Our Lady of Mercy Life Center, supporting residents in regaining strength, independence, and confidence. Our Sub‑Acute Rehabilitation Unit provides compassionate, goal-oriented care in a warm, community‑focused environment. We are hiring Per Diem RNs and LPNs with flexible commitment levels of 16 or 32 hours, offering all shifts.
Responsibilities
- Conduct comprehensive admission assessments
- Interpret clinical and psychosocial data
- Develop and communicate individualized plans of care
- Educate residents and families on care and services
- Collaborate with an interdisciplinary team
- Administer medications and monitor IV therapy
- Assist with treatments and procedures
- Collect resident data and contribute to care planning
- Provide emotional support to residents and families
- Ensure safe, high‑quality care in accordance with policies and standards
Requirements
- Current NYS RN license
- Associate Degree in Nursing or diploma from an accredited nursing program
- Current NYS LPN license
