About the Role

The Clinical Pharmacist is a licensed professional responsible for providing safe and appropriate pharmacy services in accordance with company policies, professional standards, and regulatory requirements. This entry-level position involves coordinating patient services, monitoring patient progress, and educating patients, caregivers, and staff on medication use within an alternate care setting.

Responsibilities

• Determines patient suitability for home care by collecting and assessing patient-specific information, medical condition, prescribed therapy, and prognosis.
• Designs, recommends, monitors, and evaluates patient-specific pharmacy care plans, including modifications to medication therapy and monitoring parameters.
• Completes nutritional assessments and makes appropriate recommendations.
• Modifies pharmacy care plans based on monitoring data and patient-specific information, utilizing pharmacokinetic principles and assessing laboratory values.
• Provides medication-use education to staff, other professionals, patients, and caregivers using effective techniques.
• Ensures continuity of pharmaceutical care between home and other patient-care settings through systematic communication.
• Prepares and dispenses medications following company policies, including accurate calibration of equipment, ensuring proper concentration, stability, and storage.
• Adheres to safety and quality assurance practices, including proper labeling and inspection of medications.
• Complies with state/federal laws, USP standards, and DEA controlled substance regulations.
• Manages perpetual inventory systems for controlled substances and diversifiable medications.
• Manages, maintains, and troubleshoots medication administration and related equipment.
• Participates in the reporting of medication-related incidents and equipment errors/failures.
• Provides concise and timely responses to drug information requests, analyzing literature for formulated responses.
• Attends educational seminars and participates in CEU opportunities to maintain professional competency.
• Manages investigational drug products according to regulations and company policies.
• Utilizes pharmacy support personnel effectively, prioritizing workload and organizing workflow.
• Ensures the accuracy of work performed by pharmacy support personnel.
• Complies with accreditation, legal, regulatory, and safety requirements in all activities.
• Maintains confidentiality of patient and proprietary information, observing legal and ethical guidelines.

Requirements

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• One year of experience in sterile IV admixture procedures OR knowledge of non-sterile compounding, immunizations, and medication therapy management (MTM) OR completion of an ASHP accredited Pharmacy Residency Program OR completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy OR equivalent experience as approved by the Vice President (VP) of Pharmacy and/or Sr. Vice President (SVP) of Clinical Services.
• Experience providing customer service to internal and external customers, meeting quality standards, and evaluating customer satisfaction.
• Basic level skill in Microsoft Word.
• Basic level skill in Microsoft Excel.
• Basic skill level in Microsoft PowerPoint.
• Willing to travel up to 5% of the time for business purposes (within state).
