About the Role
Schofield Care is seeking caring and compassionate employees to join their team and help fulfill their mission to deliver the area's BEST senior care. The Social Worker will assess and meet the psychosocial needs of residents and provide family intervention, assist with Medicare and Medicaid procedures, and work as part of an interdisciplinary team.
Responsibilities
- Implements a plan of care that meets the emotional, social, and psychological needs of each resident.
- Prepares a social history and provides periodic social progress notes for each resident.
- Provides counseling services for the resident and/or family members, maintaining confidentiality.
- Acts as an advocate for the resident and their family.
- Serves as a liaison between the facility and the families of prospective residents.
- Arranges for the admission of new residents in conjunction with administration and the Admission Assistant.
- Interviews, screens, and provides necessary background information on prospective residents and evaluates resident's appropriateness for placement.
- Acts as a liaison between financial assistance agencies, insurance carriers, residents, and families.
- Assists in determining resident eligibility for financial aid and helps the resident and family complete necessary placement paperwork.
- Coordinates Medicare notification procedures.
- Provides an orientation to the facility for new residents, including services, information, procedures, and resident rights.
- Obtains medical approval for Medicaid residents needing transportation to medical appointments.
- Works with the Finance Department in providing and maintaining resident personal funds.
- Completes necessary paperwork, including transfer summaries for hospitalized residents.
- Disseminates educational information to residents and families.
- Assists in developing programs that benefit residents' emotional, social, psychological, mental, and physical well-being.
- Attends weekly resident care planning meetings and makes appointments for residents/families.
- Keeps departmental records, including social histories, progress notes, psychosocial assessments, statistics, and summaries.
- Assists residents/families in discharge planning.
- Assists with personal affairs and provides personal services for residents who have no family or have family living out of town, including burial arrangements when necessary.
- Facilitates needed communication between residents, families, and staff.
- Coordinates and monitors needed services for individual residents to assure an optimal level of emotional, physical, and psychological well-being and independence.
- Involves residents, appropriate disciplines, and administration regarding matters such as bed retention, room changes, transfers, and/or discharges.
- Interprets residents' needs, behaviors, and rights by extending professional intervention to all levels of staff, suggesting positive approaches, including alternatives to the use of physical and chemical restraints.
- Coordinates and facilitates referrals of residents for needed/requested services and resources not available in the facility.
- Organizes and facilitates bereavement counseling for residents, families, and other affected individuals.
Requirements
- A minimum of a baccalaureate degree from a recognized college or university in social work or a related field.
- Willingness to stay informed of legislative changes and programs affecting Social Services, Gerontology, and Long Term Care.
- Willingness to attend appropriate programs.
- Experience in geriatrics is preferred in an institutional and/or home care setting.
- Must possess good moral character, a pleasing personality, good judgment, dependability, professional business ability, and maturity.
- Must retain appropriate information in the strictest confidence.
- Must maintain a neat appearance.
