About the Role
Join Proud Moments ABA as a dedicated Care Coordinator – ABA Services in New York, NY. You will support families and children receiving in-home ABA therapy, acting as the key liaison between our clinical team and families to ensure clear communication, seamless coordination, and a compassionate experience. The ideal candidate is empathetic, highly organized, detail-oriented, and passionate about helping others.
Responsibilities
- Manage a high-volume caseload, coordinating services across multiple stakeholders, and ensuring timely follow-up on client needs, scheduling, and service-related concerns.
- Provide family and client support through strong customer service skills, building trust, navigating sensitive situations, resolving concerns, and offering compassionate and professional ongoing support.
- Establish and maintain positive working relationships with families, clinical teams, and internal departments, serving as the key point of contact and advocate.
- Employ a solutions-oriented mindset to navigate changing priorities, address challenges proactively, and remain flexible in a fast-paced environment.
- Conduct regular quality assurance reviews with client families and the care team to ensure satisfaction and continuity of care.
Requirements
- High School Diploma or GED.
- 2+ years of care coordination, case management, or related healthcare coordination experience required; ABA experience preferred.
- Good understanding of HIPAA and its application to handling protected health information (PHI), patient confidentiality, and secure communication practices.
- Effective administration skills.
- Excellent written and verbal communication skills.
- Highly computer literate with capability in email, MS office, Smartsheet and related business and communication tools.
- Fantastic organizational and time management skills.
- Strong decision making and problem-solving skills.
- Meticulous attention to detail.
- Strong soft skills - personable, engaging, and professional.
