About the Role

St. Catherine of Siena Hospital is seeking an experienced APP/MLP Nurse Practitioner to join their primary care specialists team. This role focuses on providing advanced, high-quality healthcare in an outpatient setting, specifically within pre-surgical testing. The Nurse Practitioner will coordinate patient care, perform assessments, and ensure patients are initiated on the correct clinical pathway.

Responsibilities

• Performs physical examination and comprehensive patient assessment on pre-surgical patients.
• Collects and collates all pertinent patient information related to diagnosis and demographics.
• Initiates patient on the designated clinical pathway.
• Ensures that all tests, therapies and treatments associated with the pathway are begun.
• Demonstrates care that reflects an understanding of individual cultural and religious diversities and needs, and recognition of patient rights.
• Performs patient assessments and provides appropriate ordering of testing and medications, along with formulating an appropriate plan of care.
• Admits patients.
• Performs H&P's.
• Coordinates transfers to and from other facilities.
• Collaborates with interdisciplinary team regarding discharge planning and plan of care.
• Navigates throughout the institute, able to stand/walk for extended periods of time.
• Records and reports information.

Requirements

• Graduate of an accredited school of nursing.
• Graduate of an accredited program for Certified Nurse Practitioner.
• Current licensure by N.Y.S. Dept. of Education to practice as RN and NP.
• Master's Degree and Certification required.
• BLS required.
