About the Role

The Assertive Community Treatment (ACT) team delivers comprehensive services to individuals diagnosed with severe mental illness (SMI) whose needs have not been met by traditional approaches. These individuals often have a history of frequent psychiatric hospitalizations, emergency room visits, involvement with the criminal justice system, substance abuse, homelessness, and disengagement from traditional services. As a member of the ACT Team, the Substance Abuse Counselor II will provide intensive in-home and community-based services, utilizing a person-centered planning process for rehabilitation, case management, and support. This role also involves leading the team's integrated dual diagnosis treatment (IDDT) and conducting assertive outreach using motivational interviewing and stage-wise treatment interventions.

Responsibilities

Work collaboratively with ACT team members as part of an interdisciplinary community-based treatment team.
Provide services to clients in their homes, community settings, provider offices, and other locations.
Offer direct support to clients with psychiatric diagnoses, co-occurring substance use, chronic physical health problems, and complex social needs.
Complete the ACT Comprehensive Assessment and lead the substance abuse components.
Gather information during assessments to establish the risk level for substance use and associated harmful behaviors, and identify necessary interventions.
Lead daily and weekly clinical meetings regarding substance abuse issues and incorporate findings into service plans.
Identify appropriate treatments based on the severity of substance use diagnosis, stage of change, and immediate needs.
Recommend treatment objectives for the individual's service plan.
Provide integrated dual diagnosis treatment (IDDT) and conduct weekly IDDT treatment groups.
Run substance use support and Relapse Prevention groups.
In conjunction with a Psychiatrist or Psychiatric Nurse Practitioner, offer Medication Assisted Treatment (MAT) options.
Provide individual therapeutic support for substance use, relapse prevention, psychoeducation, and harm reduction.
Deliver therapeutic support in a person/family-centered, strengths-based, recovery-oriented, trauma-informed, and culturally sensitive manner.
Direct standards for toxicology screens in coordination with the team psychiatrist and per Assisted Outpatient Treatment (AOT) orders.
Outreach and network with inpatient/residential detox and rehabilitative treatment centers.
Act as an advocate and liaison for clients seeking emergency detox, rehabilitation care, and other community supports.
Monitor treatment, provide follow-up visits, treat co-occurring substance use disorders, and respond to crisis situations.
Provide guidance, assistance, and referrals to help clients maintain a drug-free life.
Visit hospitalized clients to coordinate care and provide advocacy.
Assist with follow-up care to support discharge planning and community reintegration.
Meet monthly required treatment contact levels, with at least 80% occurring in the community.
Complete and sign progress notes, assessments, care plans, incident reports, and other required documentation.
Provide on-site crisis assessment and management during regular work hours, both in person and over the phone.

Requirements

Master’s Degree in Social Work, Psychology, or related field.
Certified by the New York State Office of Alcoholism and Substance Abuse Services (OASAS) as a Certified Substance Abuse and Alcoholism Counselor (CASAC).

Three years of experience and/or training in integrated mental health and substance abuse assessment and treatment.
Experience with motivational interviewing techniques and stage-wise treatment.
Knowledge of Assertive Community Treatment (ACT), community integrated treatment, support services, and resources.
Eligible for full and unconditional participation in the Medicaid and Medicare programs.
Knowledgeable in MAT approaches, including screenings, assessments, and medication.
Excellent written, verbal, and interpersonal communication skills.
Demonstrated team experience.
Time management, organizational, and computer skills.
Ability to evaluate and intervene in crisis situations.
Available for crisis consultation.
Bilingual Spanish/English speaking preferred.
Knowledge of Microsoft office applications, including Excel.
Competency in written, interpersonal, verbal, and computational skills for documentation.
Regularly required to talk, hear, walk, stand, and sit.
Able to lift up to 10 pounds.
Able to operate a computer keyboard, mouse, and office equipment.
Able to travel to multiple locations as needed.
Able to work flexible hours and days, including weekends, evenings, and holidays.
