About the Role
The RN Case Manager serves as a vital link between medical services and residents/staff within the ICF/IRA program, ensuring comprehensive nursing care and overseeing medical procedures. This role involves supervising LPNs, AMAP, and Direct Support Professionals, requiring a strong understanding of OPWDD regulations and JBFCS policies. The ideal candidate will effectively supervise and collaborate with diverse populations.
Responsibilities
- Provide direct nursing care as prescribed by the physician/Nurse Practitioner, adhering to nursing standards and regulations.
- Utilize the nursing process for ongoing assessment of total health needs for individuals with developmental disabilities.
- Maintain regular direct interaction with consumers, at least weekly.
- Perform nursing assessments, including direct physical examinations, upon entry and at least quarterly thereafter.
- Develop and implement individualized nursing service plans, updating them annually or as needed.
- Coordinate acute and ongoing health services based on need and regulations.
- Monitor health, behavioral status, labs, and treatment efficacy, communicating findings to clinical personnel.
- Ensure accurate, timely, and outcome-based medical record documentation.
- Provide initial and annual training to staff on nursing tasks, health measures, infection control, first aid, and client-specific health issues.
- Train new hires and per diems on medication administration, tube feeding, and diabetic care using approved curricula.
- Conduct annual clinical performance evaluations for unlicensed direct support staff on procedures like medication administration and insulin administration.
- Maintain documentation of all training, attendance, and testing.
- Utilize electronic health record management, care coordination, and external provider interfaces.
- Actively participate in the Interdisciplinary Team (IDT).
- Perform other duties as assigned.
Requirements
- Active, unrestricted New York State license as a Registered Professional Nurse.
- Proficiency in Microsoft Office Suite applications.
- Ability to work independently and exercise sound judgment and decision-making skills.
- Excellent verbal and written communication skills.
- Ability to work in a team environment.
- Experience and ability in working with electronic health record management, electronic care coordination, and electronic health record interface with external service providers.
