About the Role
Catholic Charities Brooklyn and Queens is seeking Case Managers for its Homebase Homelessness Prevention Program in Queens. This program provides services to individuals at risk of homelessness or with prior shelter history, offering case management, mediation, advocacy, financial assistance, and workshops.
Responsibilities
- Provide case management services, assessment, and intake of participants to determine eligibility.
- Respond to telephone and walk-in inquiries, conduct pre-screenings, and provide brief and full services.
- Target, engage, and outreach to publicize program availability and services.
- Attend community meetings and events to promote the program.
- Provide advocacy, linkage, and escort services to prevent homelessness.
- Ensure regular contact with assigned cases as needed.
- Establish and maintain case records, including progress notes and database input.
- Submit timely and accurate financial assistance requests with supervisory approval.
- Guide clients on housing-related issues, including landlord/broker outreach and mediation.
- Inspect housing dwellings to ensure health and safety.
- Accompany clients to apartment viewings.
- Make referrals to appropriate internal and external services.
- Supervise bachelor and master level interns.
Requirements
- Bachelors Degree and 1 year of experience in human services required for Case Manager II.
- MS/MA degree in a social service-related field required for Case Manager III.
- Experience supervising interns is a plus.
- Bilingual skills (Spanish) are a plus.
- Ability to work flexible hours, including weekends and evenings.
- Ability to conduct home/field visits and travel throughout the five boroughs.
- Frequently lifts and/or moves up to 10 pounds.
