About the Role
Our Lady of Mercy Life Center’s Sub-Acute Rehabilitation Unit is dedicated to helping patients regain strength, independence, and confidence after hospitalization. We are seeking skilled and motivated healthcare professionals for our restorative care setting.
Responsibilities
- Deliver short-term, high-acuity care focused on rehabilitation and discharge readiness
- Collaborate with physical, occupational, and speech therapists to support patient goals
- Monitor progress and adjust care plans to promote optimal recovery
- Educate patients and families on post-discharge care and wellness
- Perform direct and indirect patient care tasks
- Assist with clerical and other support tasks to ensure quality patient care and efficient unit operation
Requirements
- Must be able to read, write and speak English
- High School Diploma or equivalent life-experience preferred
- Must be a licensed Certified Nursing Assistant in accordance with New York State
- Excellent communication and interpersonal skills
- Sincere desire to serve others
- Must be able to relay information concerning a resident's condition
- Must have patience, cheerful disposition and enthusiasm
- Willingness to handle difficult residents
- Must possess the ability to work professionally with other staff, residents, family members, and outside agencies/visitors
