About the Role

The Community Health Worker serves as a bridge between healthcare providers, behavioral health resources, and the community. This role provides support, education, and navigation services to individuals facing health challenges, utilizing a “whole person” approach to improve access to healthcare and behavioral health services, enhance community wellness, and reduce health disparities.

Responsibilities

  • Improve access to behavioral health services
  • Support individuals in navigating the healthcare system
  • Facilitate coordination of care and community collaboration
  • Maintain accurate records and ensure effective service delivery
  • Address social determinants of health

Requirements

  • Experience in community health, case management, or behavioral health services.
  • Strong knowledge of community resources and healthcare systems.

Skills

  • Ability to work with diverse populations and provide culturally responsive support.
  • Excellent communication and interpersonal skills.
  • Bilingual skills preferred but not required
  • Ability to work within the community and conduct home visits as needed.
