About the Role
The Community Health Worker serves as a bridge between healthcare providers, behavioral health resources, and the community. This role provides support, education, and navigation services to individuals facing health challenges, utilizing a “whole person” approach to improve access to healthcare and behavioral health services, enhance community wellness, and reduce health disparities.
Responsibilities
- Improve access to behavioral health services
- Support individuals in navigating the healthcare system
- Facilitate coordination of care and community collaboration
- Maintain accurate records and ensure effective service delivery
- Address social determinants of health
Requirements
- Experience in community health, case management, or behavioral health services.
- Strong knowledge of community resources and healthcare systems.
Skills
- Ability to work with diverse populations and provide culturally responsive support.
- Excellent communication and interpersonal skills.
- Bilingual skills preferred but not required
- Ability to work within the community and conduct home visits as needed.
