About the Role
Catholic Health Home Care is seeking dedicated LPNs to provide comprehensive patient care within various home settings. This role involves managing patient caseloads and collaborating with a multidisciplinary team to ensure optimal patient outcomes.
Responsibilities
- Case manage, assess, plan, develop, and implement comprehensive home health care plans for patients.
- Measure vital signs, administer IVs, change dressings, and clean wounds as needed.
- Provide skilled nursing care, preventative rehabilitative procedures, and prescribed treatments.
- Collaborate with physicians, therapy, social services, pastoral care, and home health aides.
- Educate patients and families on disease management and health promotion.
Requirements
- New York State Licensed Practical Nurse (LPN) license
- Minimum of one year of recent hands-on experience
- Basic Life Support Certification
