About the Role

The Forensic Assertive Community Treatment (FACT) program delivers comprehensive services to individuals aged 18 and up with serious mental illness (SMI) and co-occurring substance use disorders, who have had limited success with traditional approaches and have histories of criminal legal system involvement. The Temp Licensed Practical Nurse (LPN) provides medication management, administration, health education, counseling, and psychoeducation to support recovery and wellness, delivering at least 80% of services in the community.

Responsibilities

  • Provides at least 80% of medication management and health services to consumers in the community, including medication administration, monitoring, phlebotomy, healthcare coordination, and psychoeducation.
  • Completes and signs progress notes, incident reports, and other required documentation in accordance with New York State Office of Mental Health (OMH) regulations, New York City Department of Health and Mental Hygiene (DOHMH) guidelines, and Medicaid billing standards.
  • Coordinates with FACT RN and prescriber on consumers’ medication management needs and healthcare services.
  • Coordinates care with family and other community service providers.
  • Participates in daily team meetings, clinical meetings, and monthly nurses meetings.
  • Uses motivational interviewing, culturally and developmentally appropriate, recovery oriented and trauma-informed approaches when delivering services.
  • Performs other duties as required by Team Leader and senior clinical leadership.

Requirements

  • Licensed Practical Nurse (LPN) with current New York State license.
  • At least three years of experience working with adults (ages 18 and up) in psychiatric nursing, substance use treatment, or social services settings caring for individuals experiencing homelessness, justice involvement, or limited social support.
  • Knowledge of community treatment, support services, and resources.
  • Highly organized and self-motivated.
  • Proficient in computers.
