About the Role

Catholic Charities of Long Island models the love of Jesus by offering dignified and compassionate care to poor and vulnerable neighbors of every faith and background. CCLI is looking for a Nutrition Navigator with a generous heart and a desire to contribute to the well-being of others to carry out our mission on Long Island.

Responsibilities

  • Prescreen low income households for eligibility for the SNAP Program and assist them by gathering the documentation needed to complete the applications process.
  • Accompany applicants to required interview at the Local Dept. of Social Services, if requested.
  • Refer low income households to other resources in their community which could further address their specific needs.
  • Record any problems or barriers clients encounter at their local Dept. of Social Services using the forms provided by Hunger Solutions New York.
  • Conduct follow up phone calls to clients to assess their need for additional application assistance, and inquire about DSS’s final eligibility determination.
  • Maintain accurate records on files describing the types of assistance and follow up efforts provided to each household.
  • Attend Health Fairs and Community Days to provide information regarding SNAP Program benefits.
  • Distribute outreach materials related to the SNAP Program to churches and non-profit agencies.

Requirements

  • High School diploma or GED; some college preferred.
  • 5 years of working with the public including demonstrating excellent customer service skills.
  • Ability to build positive relationships with external community resource staff.
  • Detail oriented and able to maintain accurate records.
  • Availability to travel to job fairs and community events, and meet with clients on week days, evenings and weekends as needed.
