About the Role

The Residential Habilitation Specialist ensures that necessary residential habilitation is provided to assist individuals in maximizing independence and integration within the community.

Responsibilities

  • Develop Residential Habilitation Plans in accordance with services outlined in each person’s Individualized Service Plan.
  • Develop formal goals to assist individuals in acquiring skills necessary to meet their valued outcomes.
  • Provide in-service training to staff on plan implementation and data collection.
  • Write a monthly Residential Habilitation note for each individual.
  • Attend Semi-Annual and Annual ISP reviews and provide written reports on provided Residential Habilitation and progress.

Requirements

  • At least a Bachelor’s Degree in a human services field.
  • At least 2-years experience collaborating with people with intellectual and developmental disabilities.

  • Proficient writing skills
  • Proficient organizational skills
  • Excellent time management
  • Experience writing formal valued outcomes/goals
  • Experience writing Staff Action Plans
  • SCIP-R trained

Salary/Compensations

  • USD $58,500.00/Yr. - USD $59,800.00/Yr.
