About the Role
The Residential Habilitation Specialist ensures that necessary residential habilitation is provided to assist individuals in maximizing independence and integration within the community.
Responsibilities
- Develop Residential Habilitation Plans in accordance with services outlined in each person’s Individualized Service Plan.
- Develop formal goals to assist individuals in acquiring skills necessary to meet their valued outcomes.
- Provide in-service training to staff on plan implementation and data collection.
- Write a monthly Residential Habilitation note for each individual.
- Attend Semi-Annual and Annual ISP reviews and provide written reports on provided Residential Habilitation and progress.
Requirements
- At least a Bachelor’s Degree in a human services field.
- At least 2-years experience collaborating with people with intellectual and developmental disabilities.
