About the Role

The Housing Wellness Coordinator provides on-site wellness services, meeting the behavioral health and social needs of residents in Supportive Housing Programs for seniors. This role involves assessing tenants, developing wellness activities, and providing supportive counseling to help residents maintain permanent housing and age in place with dignity.

Responsibilities

  • Conduct and prepare assessments, case logs, documents case notes, and care plans for tenants.
  • Establish and maintain contacts with families, social service support agencies, and health facilities.
  • Assess residents' needs and develop and implement service plans.
  • Provide counseling, behavioral interventions, restorative services, assistance in negotiating community activities, and crisis management services.
  • Assist residents in developing meaningful service plan goals and provide supports to achieve them.
  • Develop wellness goals and activities with resident and staff input.
  • Implement and promote wellness activities.
  • Ensure the development of appropriate wellness, educational, and social programming.
  • Coordinate care for targeted critical/high-need resident cases.
  • Assist Licensed Practical Nurse with medication management.
  • Work in a team approach with building property management and maintenance staff to identify and respond to residents in need.
  • Model positive advocacy and help residents self-advocate with community service providers.
  • Complete documentation, both paper and electronic forms, in a timely manner.
  • Provide ongoing, active outreach and creative engagement of residents.
  • Respond to emergency situations on-site and provide crisis intervention.
  • Maintain contact and follow-up with service providers for residents.
  • Adhere to and enforce Agency policies regarding Confidentiality and Corporate Compliance, and maintain HIPAA compliance.
  • Accompany residents to face-to-face meetings as requested.
  • Attend staff meetings, agency meetings, workshops, classes, and in-service trainings.
  • Perform other duties as assigned.

Requirements

  • Excellent organizational, verbal, and written communication skills.
  • Computer literate, including experience with Microsoft Office software packages, local area networks, and the Internet.
  • Ability to work independently as well as part of a team.
  • Regularly required to talk, hear, walk, stand, and sit.
  • Able to climb stairs if necessary.
  • Able to operate a computer keyboard, mouse, and office equipment.
  • Ability to read printed materials and computer screens.
  • Ability to work flexible hours (evenings/weekends) as assigned.
  • Able to travel throughout the five boroughs as needed.
  • Ability to lift up to 10lbs.
