About the Role
Valence Care is seeking a Licensed Pharmacist for Prime Care Pharmacy in Brooklyn. This part-time, temporary role (approx. 6 months) blends retail and long-term care responsibilities, focusing on medication safety, accuracy, and patient care.
Responsibilities
- Review, interpret, and verify prescription orders for accuracy, dosing, drug interactions, contraindications, and safety concerns.
- Prepare and dispense medications according to New York State and federal regulations and company policies.
- Provide professional pharmacy services and medication counseling to retail customers.
- Educate patients and caregivers on medication use, dosing, administration, side effects, and adherence.
- Review and oversee medication orders for long-term care facilities.
- Verify blister packs/unit-dose packaging for accuracy.
- Monitor patient medication profiles for drug interactions, duplications, allergies, and other concerns.
- Communicate with healthcare professionals regarding prescriptions and medication therapy.
- Resolve prescription discrepancies, refill requests, and medication changes.
- Maintain accurate pharmacy records and protect patient confidentiality.
- Support daily pharmacy operations, including inventory, medication storage, and quality control.
Requirements
- Bachelor’s degree in Pharmacy or PharmD from an accredited pharmacy program.
- Current, active New York State Pharmacist license and registration.
- Strong knowledge of medications, drug interactions, and medication safety.
- Excellent communication, organizational, and interpersonal skills.
- Ability to work effectively with patients, caregivers, physicians, nurses, and other healthcare professionals.
- Previous retail, clinical, home health, community, or healthcare experience preferred.
- Experience with PrimeRx/Micro Merchant pharmacy software preferred.
