About the Role

The Physiotherapist (PT) is an integral member of the Physiotherapy service, Allied Health and clinical programs. The PT enhances patient quality of life by assessing, planning, and implementing individualized physical treatment programs to maintain, improve, or restore physical functioning and cardio-respiratory health, alleviate pain, and prevent physical dysfunction.

Responsibilities

  • Providing assessment, intervention, consultation, and education services for patients in the Stroke Early Supported Discharge Outpatient Program.
  • Collaborating with the inter-professional team in providing patient-focused care.
  • Communicating effectively with patients, families, colleagues, and external stakeholders.
  • Developing and implementing creative, comprehensive, patient-centered care plans based on evidence and Stroke Practice standards.
  • Consulting with internal and external stakeholders regularly to facilitate smooth and timely patient transitions.
  • Facilitating and leading patient groups, as appropriate.
  • Participating actively in the planning, implementation, and evaluation of the program.
  • Participating in professional and organizational initiatives supporting best practice.
  • Quality management and research; providing education to students, as appropriate.
  • Provide cross-program coverage as needed.

Requirements

  • Completion of Masters Degree or equivalent in Physical Therapy
  • Current registration with the College of Physiotherapists of Ontario
  • Membership in CPA preferred
  • Basic Cardiac Life Support (C Level) Heart Saver Plus Canada (on employment)
  • Minimum 2 years clinical experience in neuro rehabilitation required; with the Stroke population preferred
  • Excellent interpersonal and communication skills (verbal & written) required
  • Strong customer service focus and excellent problem-solving skills required
  • NDT training is an asset
  • Current UHN employees must have successfully completed their probationary period, have a good employee record along with satisfactory attendance in accordance with UHN's attendance management program, to be eligible for consideration.

Salary/Compensations

  • CAD 43.47 - CAD 54.34 per hour
