About the Role

Halcyon Home is seeking a Home Care Care Coordinator with advancement potential. This role requires strong computer skills, attention to detail, and experience in scheduling for home care or personal assistant services. The position offers competitive pay and a supportive team environment.

Responsibilities

• Schedules and documents client schedules, changes, and incidents promptly.
• Coordinates care with management and the client care team.
• Supervises and manages the assigned care team, including conducting annual onsite visits and employee evaluations.
• Completes annual client supervisory visits.
• Conducts client file audits and resolves discrepancies.
• Communicates effectively with clients, families, community liaisons, staff, and healthcare professionals.
• Responds professionally to agency and client needs.
• Demonstrates commitment and competency.
• Enhances professional growth through educational programs and workshops.
• Promotes agency philosophy and policies to ensure quality of care.

Requirements

• High school graduate or equivalent.
• Current Texas driver's license.
• One year of customer service experience preferred.
• Home Care or Personal Assistant Service scheduling experience is mandatory.
• Reliable transportation and valid, current auto liability insurance.
• Ability to travel locally.
• Must be able to work a flexible schedule and extended hours.
• Participates in the On Call and Back Up On Call Care Coordinators schedule.
• Prolonged standing and walking required.
• Ability to lift up to 50 lbs.
• Ability to make quick decisions.
• Excellent problem-solving skills.
• Ability to self-manage.
