About the Role
Provide ongoing support to individuals to promote active participation in all aspects of their lives, including self-care, health, safety, household chores, socialization, and overall well-being, ensuring a high quality of life is maintained.
Responsibilities
- Provide ongoing support to individuals to become active participants in their lives, ensuring a high quality of life.
- Ensure a safe and clean home by assisting with and participating in household chores.
- Follow all treatment plans and utilize adaptive and safety equipment to promote independence and ensure health, safety, and well-being.
- Support individuals in meal preparation by teaching concepts such as diet adherence, portion control, and cooking skills.
- Assist individuals with medication administration according to policy and procedures.
- Implement and document all treatment plans and agency materials to help individuals achieve independence and ensure their health, safety, and well-being.
- Adhere to overtime policies and procedures to ensure quality support.
- Serve as a member of the inter-disciplinary team by attending and participating in meetings to discuss progress and goal achievements.
- Provide transportation for medical appointments, shopping, recreational outings, and other transportation needs.
- Mentor new Direct Support Staff by demonstrating supportive routines and reviewing training manuals.
- Perform all other job-related duties as requested by supervisor.
Requirements
- Valid and qualified NYS driver's license.
- Ability to communicate and understand written and oral directives/information.
Skills
- AMAP/First Aid/CPR/SCIP-R Training preferred.
Experience Level
- Minimum one (1) year experience working with people with intellectual and other development disabilities preferred.
Education Level
- High school diploma or equivalent preferred.
