About the Role
Provide ongoing support to individuals, promoting their active participation in daily life activities such as self-care, health, safety, household chores, socialization, and overall well-being to ensure a high quality of life.
Responsibilities
- Provide ongoing support to individuals in daily life activities, ensuring a high quality of life.
- Maintain a safe and clean home environment by assisting with and participating in household chores.
- Follow treatment plans and utilize adaptive/safety equipment to promote independence and ensure health and safety.
- Support individuals in meal preparation, teaching concepts like diet adherence, portion control, and cooking skills.
- Administer medications according to AMAP policy and procedures.
- Implement and document treatment plans and agency materials to support individual independence and well-being.
- Adhere to overtime policies and procedures to ensure quality support.
- Participate in inter-disciplinary team meetings to discuss individual progress and goal achievements.
- Provide transportation for medical appointments, shopping, and recreational outings.
- Mentor new Direct Support Staff by demonstrating supportive routines and reviewing training manuals.
- Perform other job-related duties as requested by supervisor.
- Undergo a fingerprint/criminal background check due to regular unsupervised contact with individuals receiving services.
Requirements
- High school diploma or equivalent preferred.
- Qualified NYS driver's license.
- Ability to communicate and understand written and oral directives/information.
- AMAP/First Aid/CPR/SCIP-R Training preferred.
- Minimum one (1) year experience supporting people with intellectual and other development disabilities preferred.
- Physical ability to lift and transfer individuals (children and/or adults).
- Ability to act as second staff support during transportation, including wheelchair vehicle/lockdowns.
Experience Level
- Minimum one (1) year experience supporting people with intellectual and other development disabilities preferred.
Education Level
- High school diploma or equivalent preferred.
