About the Role

Provides case management services to patients under the care of assigned providers, ensuring case coordination, effective resource utilization, and quality of care in accordance with HRSA and Albany Medical Center standards.

Responsibilities

  • Conducts age-appropriate interviews to identify patient needs, including adjustment to illness, financial/employment issues, substance abuse, and physical/mental impairments.
  • Assesses and responds to persons in crisis, intervening to ensure safety and understanding legal responsibilities regarding domestic violence and abuse.
  • Provides education on basic HIV disease concepts, transmission, community resources, and other relevant issues.
  • Assists patients in developing self-management and health literacy skills.
  • Uses motivational interviewing to impact behavior change related to HIV transmission and medication adherence.
  • Provides supportive counseling to patients and their social supports to address needs and mobilize strengths.
  • Counsels patients in adjustment to life-threatening illness and its implications, acknowledging diversity.
  • Uses education and collaborative goal setting to help patients and families understand, accept, and follow treatment plans.
  • Informs patients of opportunities to provide feedback on program services.
  • Understands legal issues regarding domestic violence, child abuse, and suicidal/homicidal ideation and intervenes appropriately.
  • Assures annual psychosocial assessment for patients.
  • Develops, monitors, and updates the service plan annually and as needed.
  • Reviews service plans and performs follow-up to determine the quality of service provided.
  • Collaborates with physicians and healthcare team members to understand social and emotional factors affecting patient health.
  • Initiates and annually updates healthcare proxies/living wills with patients.
  • Screens patients for eligibility to entitlement programs and community resources, and refers them to appropriate services.
  • Coordinates case conferences with medical and community agencies.
  • Assists patients with retention in care.
  • Obtains and updates authorizations for release of information for inter-agency care coordination.
  • Tracks patient status for Paps, annual comprehensive exams, dental care, and other medical indicators.
  • Exchanges patient updates and plans with community case managers and provides medical outcome data.
  • Communicates patient information and updates to the AMC healthcare team using EHR tasking.
  • Refers pregnant patients to family or Part D programs and monitors their appointments.
  • Meets with or contacts newly diagnosed patients quarterly for their first year to focus on concerns, retention, adherence, education, and barriers to care.
  • Documents assessments, service plans, case conferences, and other encounters in the EHR.
  • Enters service data to the AIDS Institute Reporting System (AIRS).
  • Completes authorizations for release of information with patients.
  • Facilitates patient navigation of DHIVM and other medical systems.
  • Completes documentation within 24 hours of encounter.
  • Represents AMC/DHIVM in the community through task forces, forums, and trainings.
  • Maintains records of in-services, trainings, and continuing education.
  • Participates in ongoing supervision for professional growth.
  • Participates in ongoing process improvement initiatives.
  • Works closely with the Manager of Client Services to assure appropriate program enrollments.
  • Assures Ryan White eligibility determinations on all new DHIVM patients and annually on other patients.
  • Evaluates patient circumstances and communicates objectively and non-judgmentally.
  • Addresses interpersonal conflicts professionally.
  • Responds promptly to email and phone messages.
  • Reads and understands basic medical information.
  • Possesses knowledge of medical terminology.
  • Fluency in written and oral English.
  • Fluency in Spanish is a plus.

Requirements

  • Minimum of 1 year of documented experience in case management services to medically compromised patient populations.
  • Familiarity with community resources.
  • Ability to work in a fast-paced, interdisciplinary team setting strongly preferred.
  • Workforce members are expected to ensure that access to information is based on a “need to know” and is the minimum necessary to properly perform assigned duties.
  • Use or disclosure shall not exceed the minimum amount of information needed to accomplish an intended purpose.
  • Reasonable efforts shall be made to ensure that information is adequately protected from unauthorized access and modification.

Salary/Compensations

  • $70,068.00 - $108,605.00
