About the Role

The Child Advocate - LPN will perform healthcare coordination services for children in the Special Medical Program, ensuring they receive necessary medical, dental, and behavioral health services. This role is integral to the Special Medical Team, working closely with case planning teams and utilizing a trauma-informed approach.

Responsibilities

  • Coordinate all aspects of healthcare services for a designated caseload of special medical clients.
  • Act as a liaison for healthcare providers, pharmacies, and MMCP case managers.
  • Schedule and ensure attendance at medical/dental/behavioral health appointments, coordinating transportation as needed.
  • Monitor and address youths’ healthcare needs between routine visits, including aiding parents in securing urgent care.
  • Obtain and review medical exam records to determine further treatment recommendations, including referrals and medications.
  • Ensure timely follow-up on referrals, emergency care, and hospitalizations.
  • Conduct monthly home visits to assess the child’s medical well-being, provide direction, monitor medical care follow-up, and support resource parents/parents.
  • Coordinate closely with HSVS’s 29-I clinic, Case Planning, and Children’s Health Homes teams.
  • Develop trusting caregiver relationships with special medical children and families through clear and consistent communication.
  • Collaborate with Managed Care Liaison team to ensure appointments are scheduled with in-network providers.
  • Ensure family medical history, consents, HIPAA authorizations, and other required documentation are entered in NextGen EHR.
  • Support completion of adolescent assessments, sexual/reproductive health, gender identity, sexual orientation, and family planning consultations.
  • Complete required actions in NextGen EHR, including referrals, treatment plans, and data share reconciliations.
  • Attend trainings as directed.
  • Engage in ongoing review of departmental policies, procedures, and practices for quality improvement.
  • Perform other job-related duties as assigned.

Requirements

  • NYS Licensed Practical Nurse (LPN) required.
  • Associate degree or equivalent experience in the field required.
  • One or more years of case management experience in a healthcare environment and/or in the child welfare system.
  • Knowledge of the child welfare system and the unique complex needs of the foster care population.
  • Knowledge of foster care healthcare requirements.
  • Good understanding of medical procedures and medical terminology.
  • Experience documenting in Electronic Health Records (EHR).
  • NextGen EHR experience is a plus.
  • Excellent written, oral, and interpersonal communication skills.
  • Must be a collaborative team player.
  • Proficiency in Microsoft Office, including Outlook and Excel.
