About the Role

A Patient Care Specialist 2 (PCS2) contributes to AHF's mission by providing a friendly and helpful presence to patients, individuals seeking testing, and pharmacy patrons. This role involves greeting patients courteously, scheduling appointments, and assisting clients, aiming to deliver outstanding service that enhances AHF's reputation and encourages continued engagement with services.

Responsibilities

  • Presenting a friendly, helpful face to patients, people seeking testing or pharmacy patrons.
  • Greeting patients in a courteous and sensitive manner.
  • Scheduling appointments.
  • Assisting with the needs of our clients.
  • Offering patient intake interviews.
  • Conducting financial screenings.
  • Registering patients for Ryan White and AIDS Drug Assistance Program (ADAP) services.
  • Ensuring appropriate documentation for patient eligibility is filed and entered into the computer database.
  • Accurately collecting new patient medical history.
  • Determining tests/screenings due during medical visits.
  • Taking patient vitals.
  • Documenting all patient health-related complaints.
  • Communicating with patients between provider visits for condition updates.
  • Following through on tests/procedures ordered by providers.
  • Managing the referral process by processing specialty referrals in a timely and accurate manner.
  • Providing patients with pertinent information for timely appointment scheduling.
  • Assisting with appointment scheduling as patients require.
  • Obtaining specialist consults and scanning them into the patient’s EMR.
  • Tracking and monitoring the status and progress of referrals.
  • Notifying a provider of a patient’s lapse in completing a referral.
  • Remaining in contact with the patient to improve adherence.
  • Adding consistent value to the Healthcare Center Team, especially in high-stress environments.
  • Being flexible and professional at all times.

Requirements

  • Medical Assistant certification.
  • Valid CPR Certification from an American Heart Association-recognized vendor.
