About the Role

The Wellness Nurse provides direct and indirect care to residents/patients in a sensitive, professional, and effective manner, adhering to mandated nursing standards and company policies. This role is responsible for maintaining compliance with state and federal regulations and ensuring the highest practicable physical, mental, and psychosocial well-being of residents.

Responsibilities

  • Contributes to all aspects of medication management, including orders, administration, storage, and documentation.
  • Assures resident/patient rooms and common areas are clean and safe.
  • Ensures care specialists are familiar with residents’ individual conditions and trained to provide services effectively.
  • Continually assesses residents/patients to identify changes in condition, mood, behavior, or level of function.
  • Administers medications.
  • Provides daily recommendations and documentation for Limited Nursing license (if applicable).
  • Implements plans of care and change in condition plans.
  • Serves as the Charge Nurse for assigned shift.
  • Provides accurate and timely documentation of all care provided.
  • Assembles and maintains resident charting and records.
  • Completes accident and incident reports.
  • Maintains relationships with residents, family members, and responsible parties.
  • Assists in coordinating communication among the team, residents, and families.
  • Manages emergency situations including disasters and fires.
  • Ensures adequate and appropriately trained team members are available for each shift.
  • Performs other related essential duties as assigned.

Requirements

  • RN or LPN license
  • Knowledge of the physiology and psychology of the older adult
  • Able to communicate effectively with all levels of management, team members, residents/patients, family members, vendors, referral sources and outside contacts
  • Able to make independent decisions
  • Must be able to communicate in a warm, friendly and caring manner
  • Must be familiar with and adhere to guidelines related to the Fair Housing Act (FHA) and the American with Disabilities Act (ADA)
  • Must possess a passion to work with and around senior citizens
  • Knowledge of customer service principles and practices
  • Able to stand or walk 75% of the day
  • Able to concentrate with frequent interruptions
  • Able to work under stress and in emergency situations
  • Able to work under conditions that require sitting, standing, walking, reaching, pulling, pushing, and grasping
  • Able to talk and hear effectively
  • Occasionally lift/carry up to 50 pounds
  • Work in all areas of the community
  • Subject to reactions from dust, disinfectants and cleaning chemicals
  • May be exposed to infectious waste, diseases and other conditions
  • Use personal protective equipment and supplies when needed
