About the Role
The Navigators Program provides outreach and navigation services to vulnerable populations potentially eligible for Medi-Cal enrollment or re-enrollment. This role requires bilingual proficiency in English and Spanish and reports to the Patient Services Manager.
Responsibilities
- Develop and implement a community engagement plan to connect residents to resources through appointment scheduling and follow-up.
- Lead public education activities at clinic sites to increase knowledge of health programs, coverage benefits, and eligibility requirements.
- Develop outreach materials in Spanish and English with information about Medi-Cal services, office location, hours, and contact information.
- Collaborate with community-based organizations to inform stakeholders of healthcare opportunities and assist with the application process.
- Coordinate workshops, clinics, and community meetings, supporting residents and staff.
- Conduct weekly outreach and education at local schools, events, and high-traffic areas.
- Coordinate with other Health Education Navigators to adjust and enhance team outreach and engagement strategies.
- Participate in team meetings to evaluate and develop strategies for addressing related issues.
- Compile, develop, and maintain file notes, statistical reports, and participant records for internal analysis and grant reporting.
- Maintain professional conduct regarding confidential information.
- Demonstrate strong attention to detail and manage competing priorities effectively.
- Participate in nurturing a harmonious and teamwork-oriented work environment.
- Contact patients with lapsed Medi-Cal to assist with renewals.
- Identify and assist residents who qualify for Medi-Cal enrollment.
- Protect sensitive patient health information by adhering to HIPAA standards.
- Respond to other duties as assigned by the supervisor.
Requirements
- High School diploma or equivalent.
- Bilingual in English and Spanish.
- Strong computer skills including intermediate MS Office skills, Internet research, and experience with database systems.
- Experience completing government-funded healthcare program applications.
- Experience in counseling working-class Latinx immigrants and/or providing community social services to underserved communities.
- Commitment to social and economic justice, and knowledge of issues impacting working-class immigrants in San Francisco.
- Excellent verbal and written communication skills.
- Flexible schedule required; hours may vary and include evening work.
- Ability to work within all governing rules, regulations, and policies.
- Protect sensitive patient health information by adhering to HIPAA standards.
- Provide documentation of current immunizations, including COVID-19 and TB test results valid within the last two (2) years, and MMR immunization.

- $26.18 to $28.13 per hour
