About the Role

The Community Apartment Program (CAP) Mental Health Social Practitioner works to help members live their lives to the fullest by developing independent living skills and social connections. This role utilizes community as a tool, engaging members through group dinners and neighborhood outings to foster social connections, build friendships, and use consensus decision-making for apartment setup. The goal is to ensure members have dignified homes, feel comfortable, and connect with their neighborhood.

Responsibilities

  • Perform member engagement, social design, relationship development, continuous assessment, and therapeutic intervention within the Fountain House membership.
  • Support and develop a therapeutic community based on Social Practice and International Standards for Clubhouse Programs.
  • Utilize the community to design positive interventions for barriers such as social isolation, poor self-esteem, anxiety, symptom management, and poverty.
  • Create access to non-traditional therapeutic environments to build general skills, confidence, and support members' self-efficacy, including on-site support for employment, education, and housing.
  • Develop relationships with members and the community to support positive change, combat social isolation, increase meaningful connections, and introduce members to new opportunities.
  • Engineer environments and social interactions to create therapeutic results and develop an accessible positive community.
  • Observe and assess member experiences in naturalistic environments to create specific interventions for rehabilitation.
  • Provide daily advocacy and engagement to help members gain self-worth and purpose, offering guidance for problem-solving.
  • Provide individual and group counseling to assist members in overcoming dependencies, adjusting to life, and making changes.
  • Provide training support (vocational training) for transitional employment opportunities and maintain employer relationships.
  • Support and fill-in for members in their Transitional Employment (TE) opportunities as needed.
  • Assist members with entitlements and benefits, including food stamps, SSI/SSDI, Medicaid/Medicare, Ticket to Work/ACE/Access VR, Supplemental Need Trusts, and ADA accommodations.
  • Facilitate supported housing applications (2010E) and Single Point of Access applications, and assist members with moves into housing units.
  • Advocate for members' well-being, safety, and livelihood with supports around clinical, legal, housing, appointments, medications, and independent living opportunities.
  • Acquire proficiency in skills and tasks of the assigned unit, including safety protocols, health guidelines, and how-to guides.
  • Complete all documentation, including progress notes and outreach documentation, on time.
  • Maintain compliance with all government agency requirements, company policies, and procedures.
  • Perform periodic overnight travel to High Point Farm.
  • Work some holidays, including Thanksgiving, and some weekend and evening hours.
  • Demonstrate flexibility with staying late or arriving early to fulfill unit or member needs, and interchangeability with other staff.
  • Maintain confidentiality of records relating to members' progress.
  • Perform related duties as assigned by supervisor.

Requirements

  • Undergraduate degree or an equivalent combination of training and experience required.
  • 1-3+ years’ experience working with individuals with serious mental illness and/or hands-on experience working in a community setting preferred.
  • 1-2 years coaching and counseling experience preferred.
  • 1-2 years of vocational rehabilitation, training, or employment specialist experience preferred.
  • Ability to effectively manage and handle member concerns and behaviors.
  • Proven experience with mental health crisis management by supporting and counseling members under emotional distress.
