About the Role

The Care Company is seeking a passionate and experienced Physiotherapist (PT) to join our growing community rehabilitation team. This role offers the autonomy of managing your own community caseload with support from an experienced interdisciplinary team. It's a flexible, casual opportunity that can be structured as either an employee or independent contractor, ideal for those looking to supplement an existing caseload, build a community practice, or enjoy flexible referrals.

Responsibilities

  • Complete comprehensive in-home physiotherapy assessments within the required referral timeframe.
  • Develop individualized, evidence-based treatment plans to maximize mobility, independence, safety, and quality of life.
  • Provide follow-up visits focused on therapeutic exercise, gait training, balance retraining, mobility progression, pain management, post-operative rehabilitation, neurological rehabilitation, cardiorespiratory rehabilitation, and falls prevention.
  • Develop individualized home exercise programs and educate clients and caregivers.
  • Recommend and train clients in the safe use of mobility aids, gait aids, and assistive devices.
  • Support clients and families through education, coaching, and practical strategies.
  • Collaborate closely with Occupational Therapists, nurses, Personal Support Workers, physicians, care coordinators, case managers, and Ontario Health atHome.
  • Monitor client outcomes and modify treatment plans.
  • Maintain accurate, timely, and clinically sound documentation.
  • Communicate proactively with the office regarding caseload availability, scheduling changes, urgent referrals, and client progress.
  • Independently manage caseload and schedule while receiving administrative, scheduling, clinical, and technical support.
  • Deliver compassionate, ethical, and evidence-informed care.
  • Help shape and grow the community Physiotherapy program.
  • Contribute to new service development and Requests for Proposals (RFPs).
  • Participate in quality improvement initiatives and innovative models of community rehabilitation.
  • Mentor Physiotherapy students during community placements.
  • Support the onboarding and mentorship of future Physiotherapists.
  • Collaborate with leadership on the development of clinical resources, treatment pathways, and best practices.
  • Grow into future clinical leadership and mentorship opportunities.

Requirements

  • Registered and in good standing with the College of Physiotherapists of Ontario (CPO).
  • Experience providing home and community care, particularly with HCCSS and/or Ontario Health atHome clients, is strongly preferred.
  • Community physiotherapy experience and/or hospital discharge, rehabilitation, or transitional care experience will also be considered.
  • Demonstrated skill in mobility assessments, falls prevention, gait training, balance retraining, therapeutic exercise prescription, neurological rehabilitation, orthopedic rehabilitation, and post-operative rehabilitation.
  • Experience prescribing mobility aids and collaborating with interdisciplinary teams.
  • Strong understanding of community discharge planning, mobility supports, caregiver education, and promoting aging in place.
  • Strong electronic documentation skills.
  • Experience coordinating with interdisciplinary teams and communicating with physicians and case managers.
  • Ability to work independently in the community and produce clear, timely, clinically appropriate documentation.
  • Valid driver's licence and access to reliable transportation, or the ability to travel efficiently within the designated service area.

Salary/Compensations

  • $140 per initial assessment
  • $75 per in-person follow-up visit
  • $50 per virtual follow-up visit
  • $25 per 30 minutes of approved administrative time
