About the Role

Provides and coordinates professional nursing care for hospice patients in the homecare setting, focusing on managing symptoms and providing psychosocial support.

Responsibilities

  • Performs initial and ongoing assessment of the impact of the terminal diagnosis on patient needs.
  • Performs nursing assessments, including vital signs and pain assessment.
  • Triages patient care problems.
  • Initiates appropriate nursing procedures and facilitates movement to other levels of hospice care.
  • Develops and maintains an individualized plan of care for palliative and end-of-life care.
  • Consults with and educates patients and caregivers regarding disease process, self-care, and end-of-life care.
  • Provides psychosocial support to patients and families.
  • Provides preparation and support for patient's death, pronounces deaths, and provides postmortem care.
  • Utilizes proper bag technique in performing nursing visits.
  • Reports adverse events to team leader or quality nurse.
  • Performs POS documentation and completes all documentation according to agency protocol.
  • Complies with minimum weekly visit requirements.
  • Coordinates nursing, personal, and homemaker care for hospice patients.
  • Attends IDT meetings and coordinates patient/family care.
  • Informs the physician and team members of patient's changing needs.
  • Determines scope and frequency of services based on acuity and needs.
  • Coordinates personal care, DME, and medication/supply procurement.
  • Uses case management approach, referring to other services as needed.
  • Provides supervision of care provided by CNAs, LVNs, and homemakers.
  • Documents patient assessments, contacts, physician orders, and progress notes.
  • Ensures hospital charts include documentation after each patient visit.
  • Acts as a liaison between patient, physicians, and facility staff.
  • Works with Hospice Austin and hospital staff regarding discharge planning, DME, and medications.
  • Evaluates own needs for support and self-care.
  • Recognizes and maintains professional boundaries.
  • Assists in developing and implementing discharge plans.
  • Performs other duties as assigned.

Requirements

  • Graduation from an approved school of professional nursing.
  • Unrestricted license as a Registered Nurse by the Texas Board of Nurse Examiners.
  • Valid Texas driver's license, auto liability insurance, and safe driving record.
  • CPR certification.
