About the Role

The Physical Therapist (PT) evaluates and assesses referred patients, formulating treatment plans within a home care setting. The PT delivers therapy services collaboratively with physicians and case managers, utilizing creativity and latitude in their tasks.

Responsibilities

  • Evaluate/assess needs of referred patients.
  • Formulate treatment plans in a home care environment.
  • Provide therapy services defined in treatment plans.
  • Work cooperatively with physicians and case managers.
  • Perform a variety of tasks utilizing creativity and latitude.

Requirements

  • Graduate of an accredited Physical Therapy program (Baccalaureate, Master's, or Doctorate).
  • Current and unencumbered license to practice as a Physical Therapist in the state of employment.
  • Two (2) years of appropriate experience as a Physical Therapist.
  • Possess and maintain valid CPR certification obtained in-person with a practical skills evaluation.
  • Access to reliable transportation.
