About the Role
Social Workers at Stony Brook Medicine provide empathic care to patients, families, and visitors, applying professional principles to offer integrated social casework, including counseling, therapy, and advocacy. They plan, implement, coordinate, and document treatment for individuals with complex health and social issues, and are instrumental in addressing Social Determinants of Health and healthcare equity. Qualified candidates must demonstrate excellent communication and interpersonal skills, a strong understanding of patient care, and the ability to make ethical decisions in response to changing patient needs.
Responsibilities
- Perform psychosocial assessments of selected cases.
- Effectuate discharge planning.
- Attend rounds and coordinate with the medical team.
- Collaborate with community agencies for post-discharge patient needs.
- Liaise with the SB Outpatient division for improved continuum of care.
- Ensure compliance with documentation of Social Work interventions.
- Coordinate discharge transportation, verify insurance, and advocate with insurance companies for discharge planning and reviews.
- Assist patients with financial services, benefits, and Medicaid applications.
- Coordinate and review with insurance companies, obtaining authorizations.
- Assist patients and families with advanced directives and MOLST.
- Supervise professional social work students.
- Actively participate in performance improvement activities, including program development, data collection, and analysis on palliative care cases.
- Coordinate with Psychiatry and intervene with psychiatric patients on medicine units.
- Provide cross-coverage on units.
- Perform all other duties as assigned.
Requirements
- MSW from an accredited Social Work Education School.
- NYS License within one year of hire.
- Experience in a medical health care setting.
- Demonstrates excellent written and oral communication skills.
- Demonstrates strong Psychosocial Assessment skill set.
- Demonstrates understanding of discharge planning and long length of stay barriers and systems.
- Working knowledge of Long Island and Metropolitan NY Community resources.
- Demonstrates patient first, patient advocacy, and service excellence.
- Computer literate and understanding of data analysis.
- Understanding of community systems.
- Demonstrates ability for project management and action planning.
- Demonstrates team work and team leader focus.
- Understanding of data collection and statistics.
- Ability to cover multiple patient populations.
- Verification of degree (e.g., diploma or official transcript) is required.
- Resume/CV should be included with the online application.
- Successfully complete pre-employment physical examination and obtain medical clearance.
- Complete electronic reference check with a minimum of three (3) professional references.
- Successfully complete a 4 panel drug screen.
- Meet Regulatory Requirements for pre employment screenings.
- Provide a copy of any required New York State license(s)/certificate(s).
