About the Role
The Clinical Coordinator (CC) role is multi-faceted, supporting an efficient, effective, and comprehensive transdisciplinary rehabilitation program. The CC acts as a team lead and central point of contact to coordinate and ensure a patient's rehabilitation and community reintegration needs are met.
Responsibilities
- Manages patient services and collaborates with clinical staff to develop rehabilitation plans.
- Coordinates the ongoing provision of rehabilitation services.
- Formulates comprehensive, transdisciplinary rehabilitation treatment plans, clinical outcomes, goals/objectives, and service delivery in conjunction with the patient/care partner and clinical team.
- Completes CC evaluation and develops patient initial, progress, and discharge reports.
- Monitors patient status on an ongoing basis and completes clinical outcome tools based on diagnosis.
- Monitors patient, family, physician, and payer/case manager satisfaction with the program.
- Leads interdisciplinary case conferences and team meetings.
- Communicates Treatment Plans/Progress Reports regularly to physicians, external case managers/payors, and patient/family members.
- Schedules and facilitates interdisciplinary case conferences with the clinical team, payers/case managers, physicians, and family members.
- Communicates regularly with patient, family/care partner, payer/case manager, and physician regarding patient progress and issues.
- Facilitates acquisition of necessary equipment for functional goals with funding sources.
- Participates in medical provider meetings to report and resolve medical issues for persons served.
- Monitors projected hours of service and reports hours/changes to local leadership.
- Negotiates with funding sources for extensions of service beyond initial authorizations.
- Acquires and tracks physician orders for all provided ongoing services.
- Adheres to the Company's Code of Conduct, policies, HIPAA, Corporate Compliance programs, and professional practice standards.
- Attends, participates in, and/or conducts internal staff development programs.
- Obtains continuing education as required by Company policy and regulations.
- Maintains and enhances clinical practice skills.
- Performs other duties as assigned.
Requirements
- Minimum of Bachelor’s degree in Allied Health (OT, PT SLP), Registered Nurse, Human Services, or Rehabilitation Counseling/Psychology.
- Maintains relevant professional licensure/registration/certification in the state of practice.
- Current BLS (Basic Life Support) First Aid and CPR certifications.
- Minimum of two years' experience in a rehabilitation setting with neurologically impaired patients.
- Prior case management and/or insurance administration experience preferred.
- Demonstrates excellent organization, problem-solving, decision-making, and communication skills.
- Organized and able to function under minimal supervision.
- Has reliable transportation and is able to travel to/from multiple assignments/locations on a daily basis.
- Ability to lift fifty (50) pounds.
- Moving, lifting, or transferring of patients may involve lifting of up to 100 pounds.
- Ability to stand for extended periods.
- Requires lifting, positioning, pushing, and/or transferring patients.
- Duties require frequent reaching, stooping, bending, kneeling, and crouching.

Experience Level

- Certified Brain Injury Specialist (CBIS) preferred
- BLS (Basic Life Support) First Aid and CPR certifications
