About the Role
The Case Manager Supervisor is responsible for the overall program supervision, management, and growth of the children and adult health home care management program in Chemung and surrounding counties. This position will be responsible for the growth of this team from 3 care managers to 8+ care managers.
Responsibilities
- Provide and document weekly supervision to a maximum of 8 direct reports.
- Provide and support ongoing skills training and supervision of staff team to deliver standards of care that meet the expectations of Children and Adult Health Homes.
- Assure that subordinates are compliant with NYS DOH and Lead Health Home regulations in both field work with members and in documentation of subordinate's work.
- Complete monthly audits of assessments, care planning, and care management.
- Complete monthly statistics reports.
- Monitor care management billing to ensure that all care managers are completing billing questionnaires every month for all enrolled members.
- Actively initiate referrals for care managers to ensure caseloads for team members are at capacity and support program growth.
- Participate in mandatory and optional meetings.
- Attend inter-agency meetings and participate in local networking.
- Carry a caseload of adults and/or children as required to support the program.
- Perform all duties associated with job responsibilities.
Requirements
- Bachelors Degree in Human/Services Field.
- Minimum of three years of experience in Social Services Field or 2 years as a Health Home Care Manager.
- Relevant experience in serving children and families in child welfare, crisis situations, behavioral health (Serious mental health/serious emotional disturbance), primary health care, or social services.
- Experience coordinating and participating in team settings.
- Solid writing and verbal communication skills.
- Good documentation skills.
- Work effectively and knowledgeably across a broad spectrum of cultural, ethnic, and racial communities.
- Ability to work in an effective and focused manner when self-directed and beyond immediate oversight.
- Ability to deliver family-driven, youth-guided services.
- Knowledge of community resources and counseling/social work practices with high-risk populations.
- Requires computer proficiency including word processing skills and the ability to provide culturally competent practice.
- Must have a valid driver's license that meets The Salvation Army insurance requirements.
- Must have regular access to a vehicle to provide home visits and outreach services.
- Provide assistance and/or intervention with children.
