About the Role

The Social Worker I is expected to work autonomously while developing 1:1 relationships with residents to provide comprehensive social work services, enabling them to establish and maintain safe and independent lifestyles. WSFSSH strongly supports MSW-level social workers in obtaining their licensing, offering study groups, paying for study courses, licensing exams, and registration fees. The organization also covers CEU credits, provides weekly clinical supervision by an LCSW, and allows LMSW work experience to count towards LCSW eligibility.

Responsibilities

  • Assist residents with obtaining and maintaining appropriate medical and mental health services, entitlements, and home care.
  • Assist residents with establishing and/or maintaining family and social relationships.
  • Assist residents with budgeting and income maintenance to ensure on-time rent payment.
  • Assist residents with money management.
  • Provide crisis intervention services as needed.
  • Provide client appointment escorts as needed.
  • Provide individual counseling.
  • Assist residents in participating in socialization activities through in-house and community referrals.
  • Follow up with residents and other service providers during out-of-facility stays such as hospitalizations and/or short-term rehabilitation placements.
  • Oversee the discharge planning process for residents, including completing necessary discharge forms and/or applications to other facilities.
  • Provide group work services for residents, including recreational, educational, and/or clinical group work.
  • Participate in the intake process of prospective residents as assigned.
  • Share information about resident progress, needs, and problems with other staff as appropriate, guided by the WSFSSH Confidentiality Policy.
  • Complete non-reportable and reportable incident reports as assigned.
  • Maintain competency in software and databases as required.
  • Implement emergency procedures as necessary.
  • Attend supervision meetings, team meetings, and staff meetings.
  • Maintain thorough case records, including case notes, support plans, and psychosocial histories within the required time frames.
  • Perform other duties as assigned.

Requirements

  • MSW or MA in a human services field with a minimum of two years of social work experience.
  • Fluency in written and oral English required; bilingual is a plus.
  • Experience with older adults and people with mental illness and/or substance use is preferred.
  • Must be able to stand for extended periods of time.
  • Good written and oral communication skills.
  • Good organizational ability, time management, and interpersonal skills.
  • Desire or interest in working with and supporting an aging population.
