About the Role
The Targeted Case Manager (TCM) supports client wellness and recovery goals by implementing targeted interventions for timely, high-quality care. The TCM provides access to services that encourage clients to resolve problems interfering with independence and self-sufficiency, and to maintain themselves in the community.
Responsibilities
- Provides case management to Health Home eligible clients, adults diagnosed with severe mental illness, and adults with multiple service involvements within a year.
- Provides services to adults incarcerated for more than 30 consecutive days in the past twelve months.
- Provides individual counseling and facilitates caseload group meetings and other assigned groups.
- Provides support for improved community service linkages and referrals.
- Provides transitional care and support in navigating complex health care and social services systems.
- Provides individual and family support for clients and advocates for needed services.
- Performs on-site crisis intervention and skills teaching when other services are not provided.
- Monitors that services in the treatment plan adequately address individual needs and wellness/recovery goals.
- Conducts monitoring and follow-up activities with service recipients, their families, collaterals, service providers, and other entities.
Requirements
- Minimum 2+ years of experience in counseling clients with substance use disorders and/or mental illness.
