About the Role

Provide oversight of physician services by complementing attending physician care, acting as a medical resource to the interdisciplinary group, assuring continuity of hospice medical services, and controlling patient symptoms. Serve as a hospice champion, promoting and representing the program to various stakeholders.

Responsibilities

  • Assure hospice patients are properly evaluated for recertification of terminal prognosis.
  • Provide medical management and plan of care oversight for hospice patients.
  • Offer education and consultative assistance related to hospice and palliative care.
  • Oversee clinical aspects of the hospice and direct patient care policies.
  • Provide on-call and holiday coverage as needed.
  • Offer skilled consultation to hospice staff and community physicians on difficult cases.
  • Conduct in-service training for hospice staff and CEU offerings to community physicians.
  • Actively participate in admission, eligibility, and recertification decisions.
  • Actively participate in interdisciplinary team meetings.
  • Conduct in-person visits for symptom management and face-to-face assessments as indicated.
  • Engage in quality improvement and cost containment activities.
  • Evaluate QAPI plans and monitor for medical education needs.
  • Review and update the plan of care at least every 15 days in conjunction with the attending physician and interdisciplinary group.
  • Document care provided in the patient’s clinical record.
  • Act as primary physician for patients when referring physicians are unavailable or relinquish care.

Requirements

  • Participates in ongoing medical education activities related to hospice and palliative care.
  • Maintains controlled substances registration with state and federal authorities.
  • Has experience in hospice or palliative care and/or training in end-of-life care.
  • Not excluded from participating in the Medicare program.
  • Must have and maintain a valid Driver’s License and maintain personal automobile insurance coverage.
  • Excellent organizational, verbal, and written communication skills.
  • Computer proficiency with Microsoft Office 365 and EMR system, preferred.

Education Level

  • Holds current Board Certification as a Doctor of Medicine or Osteopathy in the state without restriction or subject to any disciplinary or corrective action.
