About the Role
Community Habilitation Workers provide person-centered supports and services, emphasizing personal choice, to individuals with developmental disabilities. They offer guidance, training, support, and encouragement to foster independence and community integration.
Responsibilities
- Provide services in home and community according to the Habilitation Plan and individual Safeguards.
- Document Habilitation services contemporaneously.
- Provide input into the development and implementation of the Staff Action Plan.
- Monitor the safety and welfare of individuals served and report concerns.
- Provide the required level of supervision.
- Assist individuals served to build positive relationships in their community.
- Support individuals served to advocate for their needs and rights.
- Provide training and encouragement for personal health care.
- Communicate health care concerns to supervisor or administrator.
- Communicate professionally with individuals served, families, team members, and the community.
- Complete all required paperwork and documentation daily.
- Encourage community integration with an emphasis on inclusion, access, community awareness, and safety.
- Attend staff meetings and participate in supervision.
- Complete all required initial and annual training.
- Conduct fire drills and document results.
- Report to work promptly and adhere to the assigned schedule.
- Work overtime as needed to ensure proper staffing ratios.
- Adhere to the NADSP Code of Ethics, Justice Center Code of Conduct, and NYS DSP Core Competencies.
- Adhere to all Agency and Program Policies and Procedures, including HIPAA and Confidentiality.
- Perform other related duties as assigned.

- Valid driver’s license and the ability to operate an agency vehicle if needed.
- Ability to perform the essential physical functions of the job including stooping, lifting, kneeling, pushing, pulling, reaching, standing, walking, hearing, listening, and communicating appropriately.
