About the Role

The CORE Services Certified Peer Specialist is responsible for delivering Peer Support and other direct care services within the CORE program, assisting individuals living with severe and persistent mental illness to reach their recovery goals. This role facilitates the utilization of natural resources and recovery-oriented principles to enhance personal and family life for long-term recovery.

Responsibilities

• Oversee and manage a caseload of members enrolled in the CORE services program, providing 1:1 support as defined by an individualized service plan.
• Assist individuals in working on goals related to community connectedness, development of natural resources, physical and mental health wellness, education, employment, self-help, and independent living skills.
• Provide services using a person-centered, goal-oriented, trauma-informed, and culturally competent approach, including advocacy, outreach, engagement, self-help tools, recovery supports, transitional supports, and pre-crisis/crisis supports.
• Assist in selecting and utilizing self-directed and evidence-based recovery tools, such as Wellness and Recovery Plans (WRAP) or Relapse Prevention Planning, to develop long-term recovery skills.
• Offer health coaching and recovery support, focusing on self-advocacy, wellness self-management, medication adherence, smoking cessation, entitlements, and daily living activities.
• Assist with and coordinate transportation to appointments, including providing travel training as needed.
• Accompany members to behavioral health, medical, and other appointments to support engagement and follow-through.
• Coordinate and communicate care planning with other service providers.
• Attend and participate in collaborative CORE team meetings.
• Act as a liaison to community services, including medical, behavioral, residential, entitlement, and other necessary services.
• Maintain compliance with all New York State Office of Mental Health policies and procedures.
• Maintain organized and accurate documents and client records using AWARDS - electronic health record (EHR) database.
• Perform related duties as assigned by supervisor.
• Produce comprehensive and descriptive progress notes including time, location, intervention, and follow-up.
• Document every service with strict adherence to deadlines.
• Complete required extensive trainings and pass post-training tests in psychosocial rehabilitation, trauma-informed care, person-centered care, motivational interviewing, substance use disorders, and mental health.

Requirements

• Working knowledge of computer software and electronic health record systems.
• Demonstrated competency in written, verbal, and computational skills for record presentation and documentation.
• Knowledge of Medicaid, Social Security, and other entitlement systems.
• Excellent interpersonal skills.
• Ability and willingness to regularly travel using public transportation throughout the boroughs.
• Flexibility in hourly/weekly schedule.
• Excellent verbal and written communication skills, including effective communication with internal and external colleagues.
• Excellent computer proficiency (MS Office – Word, Excel, and Outlook).
• Ability to work under pressure and meet deadlines while maintaining a positive attitude and providing exemplary customer service.
• Ability to work independently and complete assignments within given parameters, prescribed routines, and standard accepted practices.
• Must be able to travel within the 5 boroughs using public transportation.
• Must be able to climb stairs.
• Ability to perform essential job functions safely and successfully consistent with ADA, FMLA, and other standards, including meeting productivity standards.
• Ability to maintain regular, punctual attendance consistent with ADA, FMLA, and other standards.

Salary/Compensations

• $24.68 per hour
