About the Role
As a Physical Therapist (PT) at Adaptive Home Health, you will be instrumental in restoring patient function, ensuring their safety at home, and driving measurable outcomes within a patient-centered home health model. You will leverage technology designed to reduce documentation burden and streamline visit coordination, allowing you to focus on delivering exceptional care.
Responsibilities
- Own caseload as a case manager, accountable for patient therapy plans, progression, and outcomes.
- Complete comprehensive in-home assessments and deliver skilled PT visits.
- Build and execute clear plans of care including gait/balance, transfers, strengthening, functional mobility, home safety, and fall prevention.
- Set measurable goals, track objective progress, and adjust treatment as needed.
- Drive multidisciplinary coordination by communicating changes in condition and aligning on orders.
- Manage treatment and outcomes, including PTA supervision where appropriate.
- Coach patients and caregivers with practical home programs.
- Document promptly and thoroughly in Homecare Homebase (HCHB).
Requirements
- Active Texas PT license in good standing.
- Strong clinical judgment and comfort working independently in the field.
- Comfortable escalating changes in condition to RN/MD and documenting rationale clearly.
- Strong communication and documentation skills.
- Reliable follow-through on orders, frequencies, and patient goals.
- Current driver's license and comfort with daily travel across assigned territory.
- Able to carry/transport basic therapy equipment as needed.
- Competency in core home health PT scope.
- Proficiency in safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations.
- Experience with strengthening, functional mobility, and home safety assessment/modification education.
- Experience in patient/caregiver coaching with practical home programs.
- Prior home health experience is preferred.
- Homecare Homebase (HCHB) experience or similar home health EMR experience is preferred.
- Deeper clinical experience in high-signal PT populations is preferred.
- Experience supervising/mentoring PTAs and collaborating on care plans is preferred.
- Strong organization and time management skills are preferred.
