About the Role
Professional Care Medical Practice P.C. is seeking a Licensed Mental Health Counselor (LMHC) or Licensed Social Worker (LMSW or LCSW) to serve as a Behavioral Health Professional within the medical department at the Onondaga County Justice Center in Syracuse, NY.
Responsibilities
- Conduct clinical assessments, including documentation, case conceptualization, diagnosis, and treatment planning.
- Make referrals to psychiatry as needed.
- Perform risk assessments using structured interviews and tools, managing inmates at risk for suicide or self-harm according to protocols.
- Act as the PCM mental health liaison for external mental health services.
- Facilitate admissions and placements to forensic hospitals when required.
- Monitor all provided mental health services and ensure adherence to NCCHC and accepted psychiatric/psychological standards.
- Participate as a member of the Quality Assurance Committee.
- Develop and conduct in-service education programs.
- Collect data and prepare monthly performance reports.
Requirements
- Must be licensed as a LMHC, LMSW, or LCSW in New York State.
- Must hold a Master’s degree or higher in Social Work, Psychology, or a related Human Service field with a clinical concentration.
- Minimum of two years of experience and familiarity with substance abuse/MAT counseling is preferred.
- Must possess a valid driver's license and reliable transportation.

Benefits
- Part Time PTO package provided and accrued at the rate of 1.0 hour for every 30.0 hours of straight time worked.
- Maximum PTO accrual of 64.0 hours.
