About the Role

Professional Care Medical Practice P.C. is seeking a Licensed Mental Health Counselor (LMHC) or Licensed Social Worker (LMSW or LCSW) to serve as a Behavioral Health Professional within the medical department at the Onondaga County Justice Center in Syracuse, NY.

Responsibilities

  • Conduct clinical assessments, including documentation, case conceptualization, diagnosis, and treatment planning.
  • Make referrals to psychiatry as needed.
  • Perform risk assessments using structured interviews and tools, managing inmates at risk for suicide or self-harm according to protocols.
  • Act as the PCM mental health liaison for external mental health services.
  • Facilitate admissions and placements to forensic hospitals when required.
  • Monitor all provided mental health services and ensure adherence to NCCHC and accepted psychiatric/psychological standards.
  • Participate as a member of the Quality Assurance Committee.
  • Develop and conduct in-service education programs.
  • Collect data and prepare monthly performance reports.

Requirements

  • Must be licensed as a LMHC, LMSW, or LCSW in New York State.
  • Must hold a Master’s degree or higher in Social Work, Psychology, or a related Human Service field with a clinical concentration.
  • Minimum of two years of experience and familiarity with substance abuse/MAT counseling is preferred.
  • Must possess a valid driver's license and reliable transportation.

Benefits

  • Part Time PTO package provided and accrued at the rate of 1.0 hour for every 30.0 hours of straight time worked.
  • Maximum PTO accrual of 64.0 hours.
