About the Role
Health Quest Home Care is seeking a Home Health Aide to assist patients with personal care, exercises, meal preparation, medication prompts, and monitoring their condition. This role requires identifying changes in patient status and taking vital signs.
Responsibilities
- Assists the patient with personal care activities including bathing, oral hygiene, feeding and dressing.
- Assists the client with exercises, ambulation, and transfer activities, including the use of assistive devices.
- Independently assists the patient with meal preparation/cooking and/or feeding.
- Must have working knowledge of special diets, diet restrictions, and techniques to maintain sanitary kitchen conditions.
- Assists the patient with medication prompts to remind the patient to self-administer medications.
- Identifies changes in patient condition that require notification to the patient’s registered case manager.
- Takes vital signs to include temperature, pulse, heart rate, respiratory rate.
Requirements
- Active NYS Home Health Aide Certification.
- Driver's License.

- Significant manual skills / motor coordination & finger dexterity
- Significant occupational risk
- Significant exposure to dirt, odor, noise, human waste, etc.
- Working knowledge of special diets, diet restrictions, and techniques to maintain sanitary kitchen conditions to prevent food-borne illnesses.
- Knowledge in use of assistive devices such as canes, walkers, crutches, and lifts.
