About the Role
We are seeking career changers for support services and individual case assistance at our 'Wilhelmine-Striedieck-Haus' facility in Bernau OT Lobetal, focusing on participation and support.
Responsibilities
- Maintain and enhance residents' quality of life and satisfaction through individualized support and care.
- Structure daily routines, ensure daily structure, and manage the living environment based on individual needs.
- Independently prepare, execute, review, and update support, development, and care plans, considering participation plans after professional training.
- Perform necessary household and care tasks, involving and guiding individuals with disabilities.
- Maintain PC-based support and care documentation.
- Provide assisted support to beneficiaries in daily living.
- Assist in creating a living and life environment appropriate to the beneficiary's needs, exploring social environments, facilitating leisure activities, and fostering social contacts.
- Assist in executing medical measures as per doctor's orders, including medication, medical and nursing care, and doctor consultations/appointments.
- Actively contribute to assessing potential risks of harm to beneficiaries and third parties in cases of imminent danger.
- Actively implement individual support plans and evaluate their effectiveness.
Requirements
- Knowledge and experience in supporting, assisting, and caring for people with disabilities are desirable.
- Willingness and ability to collaborate within a team and to independently plan and execute person-centered tasks, especially for individual case assistance.
- Willingness to work in shifts.
- Submission of an extended police clearance certificate and a valid health certificate.
- Applicable PC skills.
- Driver's license Class 3/B is desirable.
