About the Role

Conifer Park is seeking clinicians to provide outpatient treatment and cutting-edge support for individuals struggling with substance-use-disorder (SUD) in Syracuse, NY. Clinicians will offer comprehensive case management, including assessments, individual, group, and family therapy, treatment planning, discharge planning, and intervention with client support systems.

Responsibilities

  • Provides comprehensive evaluation of patient's functioning.
  • Coordinates the development and implementation of the multidisciplinary treatment plan and reviews within established time frames.
  • Documents patient progress and interventions within regulatory time frames.
  • Provides individual, group, and family treatment (psycho-education, life skills) as identified in treatment plans and requested by program director.
  • Performs utilization review, provides progress reports, and develops aftercare plans.
  • Participates in quality improvement activities.
  • Maintains professional competence in addiction medicine through continuing education and supervision.

Requirements

  • CASAC-T, CASAC certification or LMSW, LMHC required.
  • Experience working with outpatient substance abuse counseling preferred.
  • Bilingual language speakers preferred.
