About the Role

GGRC is seeking Social Workers - Early Start Assessment to assess applicants (0-36 months) for the Early Start program. This role involves facilitating IDCT eligibility assessments and developing Initial IFSP within 45 days of referral.

Responsibilities

• Conduct initial Early Start applicant assessments, including client/family interviews and needs assessments.
• Design, monitor, and revise Individual Family Service Plans (IFSP).
• Confirm immediate needs and arrange for necessary interventions or services.
• Complete Early Start Assessment reports and Applicant Developmental Evaluation Reports (CDER).
• Schedule and coordinate meetings with assessment teams and applicants.
• Provide information and referrals for ineligible applicants.
• Research community resources and ensure services comply with the Lanterman Act and Title XVII.
• Maintain applicant records, case notes, and progress notes.
• Prepare written and verbal reports and handle miscellaneous paperwork.
• Participate in staff, community meetings, and task forces.
• Assist community resources in improving service quality.
• Identify community service needs or gaps and stimulate service development.
• Maintain applicant information confidentiality in compliance with HIPAA and the Lanterman Act.

Requirements

• Bachelor's degree in social work with 1 year of experience working with the 0-36 month population or related field.
• 2+ years (post-doctoral) work with the developmentally disabled population.
• Access to reliable transportation is needed for local travel within San Mateo County.
