About the Role

The Foster Care Caseworker will collaborate with the Foster Care Department, County Department of Social Services, and family members to fulfill contract requirements. This role involves providing Foster Care Case Management, assessing client needs, and participating in visitation, safety, treatment, and discharge plans. The Caseworker will also offer supportive counseling, complete assessments, and connect individuals and families with necessary resources.

Responsibilities

Assist and support foster youth with their safety, well-being, educational, medical/mental health, and family needs.
Provide services and support to birth families to achieve permanency goals.
Ensure job-related activities comply with Glove House policies, OCFS, State and Federal regulations, and professional standards.
Perform Foster Care Case Management, including treatment planning, monthly contact with schools/therapists/county CW, engaging biological families, permanency planning, independent living skills, family and sibling visitation, and face-to-face contacts.
Provide or arrange transportation for youth to appointments and visitation.
Participate in the Foster Care Department team and assist with recruitment efforts and presentations.
Support and provide feedback to discharge resources regarding their strengths and needs for reunification, monitor court-ordered stipulations, and address barriers to permanency.
Complete monthly home-visits of discharge resources.
Provide supportive casework counseling services for foster parents, including education and crisis management.
Provide case management services including referrals, connections, and links.
Train direct care team members on foster care management contract components.
Participate in foster youth treatment team meetings.
Generate and maintain necessary reports and paperwork (e.g., Quality Assurance and program reports).
Ensure timely documentation of all contacts and situations in myEvolve and CONNECTIONS.
Utilize the agency EMR, myEvolv, to accurately represent the youth’s record.

Requirements

All degrees must be from an accredited university.
Minimum 3+ years’ experience working with children and families in a residential, group, or counseling child welfare capacity.
Must have a valid driver’s license.
